  • Yes. To see Dr Kozlowska as a specialist, you will need a current referral from your GP or another specialist. Medicare requires a valid referral for specialist rebates to apply. Please bring your referral letter to your first appointment.

  • Please bring your referral letter, Medicare card, a list of current medications, any recent investigation results (blood tests, imaging, ECGs), and any relevant previous specialist letters.

  • You can contact our practice directly by phone or email. Once we have received your referral, our team can assist with booking an appointment.

  • Appointment times can vary depending on the reason for review and the complexity of your care. Our team can advise you when your booking is made.

  • Yes. We regularly provide telehealth follow-up appointments after your initial consultation, where appropriate, to review your progress and discuss ongoing care.

  • If you have a valid referral, a Medicare refund will be apply to your consultation and is processed on the day of your appointment. The out-of-pocket cost will depend on the type of appointment.

  • Payment is required on the day of your appointment.

  • Yes. A letter outlining the consultation and management plan will be sent to your referring doctor and, where appropriate, your regular GP.

  • Follow-up appointments can be arranged through our rooms after your consultation.

  • If your doctor considers the referral urgent, they should clearly mark it as urgent and contact our practice directly if needed.

  • Please contact us as soon as possible if you need to cancel or reschedule your appointment. This allows us to offer the appointment to another patient. Cancellation policy details can also be provided by our team.
